AND NOW, this 12th day of September, 1994, IT IS HEREBY ORDERED that any person whose name does not appear on the Master List of Prospective Jurors and who meets the qualifications for jurors set forth by law and who makes application to be listed on the Master List of Prospective Jurors shall be included in the Master List of Prospective Jurors. See 42 Pa.C.S.A. §4521(a). For purposes of making application to be included on the Master List of Prospective Jurors, the attached form shall be used.
